Here's some basic information for you;

Group Health Insurance - Group Health Insurance is a health insurance policy purchased by a company or organization that covers its employees. A group health insurance policy is underwritten on the health experience of the entire group and not the specific individuals in the group. After a group health insurance policy has been approved and is in place for the employer or organization then all employees who meet the employers eligibility requirements will be accepted regardless of their individual health condition.

Employees insured under a group policy are all covered by the same provider network and all have access to the same level of coverage, deductibles, co-pays and other policy benefits. Monthly premiums are paid by the employer directly to the health insurance company. While the employer is responsible for paying the premium, the cost may or may not be shared between the employer and employee. Some employers pay the employees premium at a 100% level while others may share the cost with their employees. Most companies will require the employer to pay at least 50% of the employees premium.

The employer is the owner of a group health insurance policy. Because of this, employees who may leave employment are no longer eligible for coverage and will be removed from the group. Employers with 20 or more employees will be required to offer terminated employees an 18 month COBRA option. Under the COBRA option a terminated employee will be allowed to continue coverage at their own expence.

Here's some typical group health insurance eligibility requirements;

  • Your company has at least two full time employees
  • At least 75% of all eligible employees are participating in the group
  • Your company pays a minimum of 50% of the employee premium cost



Major types of group health insurance

  • (HMO) Health Maintenance Organization
    • Generally the lowest cost program for employers. A HMO may be a single group or network of providers. The employee must choose a primary physician who guides all health care under the plan for the employee. It's through the primary physician that all employee medical care begins. The primary care physician provides referrals to all specialists that may need to be called upon to provide services. HMO's are not very flexible. They are usually limited to certain geographical areas.


  • (PPO) Preferred Provider Organization
    • PPO's are far less restrictive that HMO's. They provide coverage over broader geographical areas. The PPO is a managed care network of providers that allows the employee access to any network provider. No referral from a primary care physician is required. Employees may even seek care outside of the network however at a reduced benefit level.


  • (POS) Point of Service
    • A hybrid of the HMO and PPO concepts. Employees must choose, as in the HMO, a primary care physician. If an employee uses the in network benefits the policy responds more like the HMO. Employees may also use out of network but will be subject to a higher participation level through deductibles or copayments.
